Procedure for Obtaining Your Win / Loss Tax Statement

Win / Loss Tax Statements that document your overall win or loss will be provided at your request by following the simple steps outlined below:

  1. Visit a Gamblers Bonus location where you frequently play. We suggest one of the many local taverns nearest you.
  2. Tell one of the location's staff that you would like to have your name put on the sign up sheet for a win / loss statement. He or she will require your full name, 6-9 digit log on number and the calendar year of the statement being requested. Do not include your 4-digit PIN number.
  3. The location will then fax the sign up sheet to our office where we process your request as quickly as possible.
  4. Each statement is inserted into a sealed, confidential envelope and delivered back to the location.
  5. You will be able to retrieve your statement at that location after providing photo identification.

Please note:

You only need to request your statement one time at one location. This statement will contain all your gaming transactions at all the various locations where you have played during the last year.
